Before You Begin
Please read the information below before you begin.
Before you begin please collect all the materials for your submission. A pre-submission checklist is below:
- Your success story, as a Word document or text file.
- A video file or YouTube link if you are submitting a video.
- Any images you would like to include.
- Release forms (see below).

In order to share your story you must complete one of the a release forms and upload it with your submission. Please download and complete your release form before starting the submission process.
